Choose the Correct Form
1. Encounters / Abductions Form
Use this form for any direct interaction with non-human entities, missing time, reproductive procedures, family threats, memory suppression, or close-contact events.
- Record exact times, locations, and sequence of events
- Describe physical appearance, height, skin texture, eyes, clothing (or lack thereof), and any vocalizations exactly as observed
- Note physical sensations (paralysis, buzzing, pressure, temperature changes) without guessing their cause
- Include details of any higher/commanding presence if observed
2. UAP / USO Sightings Form
Use this form for any unidentified aerial phenomena, submerged objects, or craft sightings without direct entity contact.
- Record exact time, duration, direction, altitude, speed, and maneuvers
- Describe shape, size, lights, color, sound (or silence), and any environmental interaction
- Classify clearly as Aerial, Submerged, or Terrestrial
Emergency Alert Checkbox – Critical Safety Feature
Both forms include a single Emergency Alert checkbox near the bottom.
When to check this box:
- You or someone else is in immediate physical danger
- An active abduction or close-contact event is occurring or has just occurred
- You have credible reason to believe a repeat event is imminent
- You observe a grounded craft with entities present and believe public safety is at risk
What happens when checked:
- Real-time email alert is sent to all registered users in your selected Home County
- A national-level alert is issued simultaneously to the broader network
- Level-1 Emergency Moderators are notified immediately
Only check this box when the situation truly requires immediate community awareness.
Final Steps Before Submitting
- Review every field for accuracy and objectivity
- Attach only evidence obtained safely from a distance (photos, video, audio)
- Select your Home County for alert routing
- Decide on the Emergency Alert checkbox based on the safety criteria above
- Submit the report
